On our desk, the valuation debate on GMED is centered around its stellar balance sheet versus slowing earnings velocity. With a debt-to-equity ratio of 0.02, $630M in cash, and $341M in annual free cash flow, the financial distress risk is practically zero. However, the market is grappling with a forward EPS growth deceleration down to 8.5% from historical levels near 66.7%. At a P/E of roughly 17 to 22 depending on TTM vs forward views, this creates an implied PEG around 2.2 that caps near-term multiple expansion. We are keeping an eye on whether management's strategic flexibility from the NuVasive and Nevro deals can reignite growth before investors rotate out.

Looking at the Q2 2026 print, worldwide net sales hit $789.6 million, representing 6% overall growth or 9% excluding Nevro. But as a bio-analyst tracking the integration metrics, the structural changes made within the Nevro sales and marketing organization at the tail end of 2025 are acting as a temporary anchor. Leadership explicitly warned that Nevro's performance 'will probably get a little bit worse before it gets better.' Additionally, a sequential revenue step-down in Enabling Technologies and a market shift toward leasing versus cash sales could introduce volatility to top-line performance moving through the back half of 2026.

In our clinical protocols and technology evaluations, Globus continues to capture share across musculoskeletal, spine, and enabling tech portfolios backed by nine new product launches in 2025. Surgeons frequently note the utility of their surgical robotics and the HFX spinal cord stimulation platform gained via Nevro. However, when building clinical adoption models, we have to account for execution friction during commercial integration phases. Trailing 5-year EPS growth of 12.6% still lags the broader medical device industry average of 18.4%, meaning product differentiation must successfully offset integration headwinds to justify premium valuations.
